I approve the chain, metal, and fit before welding. I understand the fit can't be changed after welding without cutting the chain.

Welding Safety

I understand the welder makes a small, quick spark and flash of light, with brief heat at the connecting ring.
I understand a protective patch or shield will be placed under the weld point to protect my skin. I will hold still during welding.
I will look away or wear the eye protection given to me during welding.
I understand I may feel brief warmth, and a small burn or mark on the skin is possible.

Metal Allergies

•Known metal allergies (such as nickel)?
Yes, describe: [Allergy Details]
No
I understand that even quality metals can contain small amounts of other metals. If I have a reaction, I will have the chain removed and see a doctor if needed.

Removal

I understand the chain can be removed quickly by cutting the jump ring (the small connecting ring) with scissors or wire cutters.
I will remove the chain, or let medical staff remove it, before an MRI, surgery, X-ray, or other procedure if asked. I understand medical staff may cut it off in an emergency.
If the area swells from an injury, pregnancy, or allergic reaction, I will have the chain removed.
I understand the studio's re-weld policy: [Reweld Policy]

Catching Hazard

I understand the chain can catch on machinery, sports gear, clothing, hair, or pets. Catching can cause injury or break the chain. I will take care, or have it removed, for activities where this is a risk.
I understand some jobs have rules about wearing jewelry. Checking those rules is my responsibility.

Care and Wear

I understand chains can wear, stretch, tarnish, or break over time, depending on the metal and how I wear it.
I understand lotions, chlorine, and salt water may affect the finish of some metals.

Refunds

I understand that once the chain is welded, it's custom fit and the sale is final. No refunds are given after welding, including if I cut it off later.
I understand the studio's repair policy: [Repair Policy]

Tips for using this form

  • • Keep wire cutters at your station, and show clients where the jump ring is
  • • Ask about metal allergies before the client picks a chain
  • • Have clients approve the fit before you weld, since you can't adjust it after
  • • Post your no-refund and re-weld policy where clients see it before they choose
  • • Ask about work rules and upcoming medical procedures that may mean the chain has to come off

Frequently Asked Questions

Do you need a waiver for permanent jewelry?

It’s a smart idea. A waiver records that the client understood the welding process, metal allergy risks, how to remove the chain, and your refund policy. It sets clear expectations and gives you a signed record if questions come up later.

What should a permanent jewelry waiver include?

The piece and metal type, fit approval, weld safety notes, a metal allergy question, how to remove the chain in an emergency or before medical procedures, the catching hazard, your refund and re-weld policy, and signatures.

Can you take off permanent jewelry?

Yes. Cut the jump ring, the small ring that connects the chain, with scissors or wire cutters. It comes off in seconds. Medical staff can do this too. Show clients where the jump ring is before they leave.

Can you wear permanent jewelry during an MRI or surgery?

Medical teams often ask patients to remove metal jewelry first, so follow their instructions. The chain can be cut at the jump ring. Some studios offer a re-weld afterward, so state your re-weld policy on the waiver.
